Long Term Home Health Care Program + AIDS Home Care Program LTHHCP

The Long Term Home Health Care Program (LTHHCP) is primarily for Medicaid eligible persons addresses the needs of the frail elderly, and the chronically ill and disabled of all ages who can be maintained at home with appropriate services over an extended period of time. This program, also known as "Nursing Home Without Walls", is a joint case management program with Department of Social Services and DSFHS that maintains persons eligible for nursing home placement at home at 75% of the cost of an area nursing home.

AIDS Home Care Program
The AIDS Home Care Program is a Long Term Home Health Care Program for persons living with AIDS. It provides all the services of the LTHHCP without a cap. Care is complex and challenging, requiring excellent case management. Education programs and mental health consultation are available to meet patient and staff needs.

Outreach services are offered at all of our facilities, in Westchester, the South Bronx and Suffolk County.
